Describe your dog- name, age, breed etc Why did you choose this breed - or this particular dog/ puppy from the litter or how did the dog come to live with you?

1   How long have you had your dog.


2   Is this your only dog.


3   Who lives in your house. (if children then please state ages)


4   What is your routine with regards to your dog - ie do you give her a walk in the afternoon, when do you feed her etc.


5   When you get back home/ after any separation/ what does she do - what's her reaction.


6   What do you do when you return? (with regards to your dog)


7   Explain how you feed her and when - before or after you eat? Is she possessive over food, what is her reaction if you walk by her when eating and has she ever growled at you in relation to food.


8   What does she enjoy playing.


9   Does she have toys in the home that she can play with when she wishes?
If so does she bring them to you when she wants to play?


10 Are there no go areas of the house or does she have full access. This may change at certain times of the day/ night. Be specific.


11 Where does she sleep at night. Do you have any sort of ritual/ routine?


12 Does she have a favourite resting place during the day- does this change when you move around in the house.


13 Is she generally around you when you are home - does she follow you?


14 What happens when someone knocks on the door.


15 What happens when people enter the house?


16 If you open the door does she try to go out first?


17 What does she do when you are eating?


18 Where does she lie when the family are in the living room/ all together.


19 Does she have a favourite resting place?


20 Does she ever exhibit what humans would term jealousy or possessiveness?
If so describe the circumstances


21 How is she on the lead and out walking - do you let her off or keep her on a lead.


22 What is her reaction to other dogs?


23 What is her behaviour towards other people- those she knows and those she doesnt?


24 What is your reaction if she acts 'inappropriately'. How do you scold/ discipline. Give examples where possible.


25 Does she bark- if so, when? Is this a problem? How do you react?

 

26 Lastly, give a description of your dog's personality and any other details you think would be relevant to your problem/s
